Knowing when the runway is clear for take off...

Say you're 4th in line on the runway for takeoff. You can't see over your nose so how do you tell when the runway is clear?

Do you turn your plane slightly to the side so you can see?

Sit and listen for the first 3 engines to rev up and then go?

Continuously ask ground control for takeoff clearance?

Or something else?

Normally you would line up alternating in left and right of center-line. Close enough so you can see part of aircraft in front of you so that even the leader has maximum amount of runway available. You can turn plane slightly to see, but that might not help much with a big radial engine in front. Tower might hold the formation short of runway, and when you take position on runway you can see your relative spot.

The problem seems to be that at least some of the campaign missions start with you in a line down the centre of the runway.
